How to report an adverse event
To report an adverse event or a concern about an Origyn Labs product, contact us using the information above and include the following:
- Product name and SKU
- Lot number, from the product label or the QR code
- Date of the event
- A description of what occurred
- Your contact information, so we can follow up
What we do with a report
We log every report, review it against the lot's laboratory results, and determine whether the report points to a single lot or to a broader concern. We follow up directly with the reporter when we need more information.
Recall procedure
A recall at Origyn Labs is lot-level. If a lot is recalled, we notify wholesale buyers who received that lot and request the distribution records described in our Wholesale Terms — the lots received, quantities, and downstream businesses or locations — so the recall can reach every affected unit.
Lot certificates
Every lot has a public laboratory certificate at /coa/. No login is required to view it.
Reporting to FDA
Consumers and healthcare professionals may also report an adverse event to the FDA through its MedWatch program.
